Enhanced Care Management (ECM):
- Provides comprehensive care coordination for Medi-Cal beneficiaries with complex needs.
- Addresses medical, behavioral, and social determinants of health.
- Includes care coordination, health education, linkage to resources, and personalized care plans.
Housing:
- Focuses on housing needs of vulnerable Medi-Cal populations.
- Aims to secure and maintain stable housing for the homeless or at-risk individuals.
- Includes housing transition navigation, tenancy-sustaining services, and collaboration with housing authorities.
Asthma Remediation:
- Targets the improvement of living conditions for individuals with asthma.
- Includes home assessments, removal of asthma triggers, and education on asthma management.
- Aims to reduce asthma-related health issues and improve overall quality of life.
