CalAIM Enhanced Care Management (ECM) is a free Medi-Cal program that pairs you with a personal Lead Care Manager. Together, you will set goals and connect to the medical, mental health, housing, and social services you need.

What is Enhanced Care Management?

Enhanced Care Management (ECM) is a free Medi-Cal program that provides extra, personalized support, especially if you have complex health needs. You will work closely with a Lead Care Manager (LCM) who gets to know you and helps coordinate every part of your care, so you can feel more in control of your health.

Your Lead Care Manager, or LCM, is one person who helps bring your whole care together. They take time to know you, help you set goals that matter to you, and connect you to the medical, mental health, housing, and social services you need. Your LCM can meet you by phone, in person, or where you already get care, so support fits into your life.

Help moving into a new home

A one-time benefit that helps eligible members cover the upfront costs of moving into a new home. Funds and approved items support your health, safety, and housing stability, as outlined in your Individualized Housing Support Plan.

📋 How it works

Not covered: ongoing rent, room and board, and non-essential or personal items. All purchases must be medically or functionally necessary and listed on the approved Essential Items List, or covered by a medical waiver. Payments go directly to landlords or vendors and require a W-9 form.

CalAIM Eligibility

Who qualifies?

ECM and CalAIM Community Supports are for Medi-Cal members with the most complex health needs, such as people experiencing homelessness, serious mental health or substance use challenges, or frequent hospital visits. To take part, you need Medi-Cal through a managed care health plan, and your health plan makes the final decision on who qualifies. Not everyone will be eligible.

How does CalAIM work?

How to get started

ECM is for Medi-Cal members with the most complex health needs, and your health plan makes the final decision on who qualifies. You do not need a long application to find out if it may be right for you. To join ECM, you need Medi-Cal through a managed care health plan and a referral to that plan. Your care team can start this for you, and so can you.

The best way to begin is to call us. We will talk about your needs, help start a referral if it makes sense, and walk you through each step in your language.

Steps:

  1. Call us. We will talk about what is going on and whether ECM may be a good fit.
  2. We start your referral. With your okay, we send a referral to your Medi-Cal health plan.
  3. Your plan reviews it. Your managed care health plan makes the final decision on who qualifies.
  4. You meet your Lead Care Manager. If you are approved, your LCM begins working with you right away.
How do I know if I am eligible?
ECM is for Medi-Cal members with complex health needs. Call us at 714-503-6550 and our team will help determine if you qualify.
What does a Lead Care Manager do?
Your LCM is your personal guide. They help you set health goals, connect to services, coordinate your care, and build the skills to manage your health more independently.
Is the Housing Deposits Program cash?
No. The benefit is paid directly to landlords or vendors, not given as cash. It covers approved costs like security deposits, utility setup, and essential household items.
Can I use the Housing Deposit benefit more than once?
No. It is a one-time, once-in-a-lifetime benefit designed to help you move into a new home.
How do I get started with the Housing Deposits Program?
Work with your Lead Care Manager to complete a Housing Support Plan. Your LCM will guide you through exploring housing options and making sure you have what you need to feel settled.
