What to expect at your first visit

Plan for about 60 minutes for your first visit, including check-in and time with your provider. 

💡 Please complete your forms online before arriving to save 15 to 20 minutes at check-in.

Check in at the front desk

Staff will verify your ID, insurance, and completed forms. If you haven’t completed your forms online, plan to arrive 30 minutes early to complete them at the front desk.

Vital signs and health history

Blood pressure, weight, temperature, and medications recorded before your visit begins.

Meet with your provider

Review your health history, discuss concerns, and complete a physical exam. 

Create your care plan

Outline next steps together, including labs prescriptions, referrals, and follow-up schedule.

Under federal law, uninsured and self-pay patients have the right to recieve a cost estimate before treatment.

Common questions from patients

How do I apply for the sliding fee scale?
  1. Complete the application. Fill out the Sliding Fee Assessment Application online in your preferred language, or pick up a paper copy at any KCS location.
  2. Provide your documents. Bring or submit your proof of income, ID, and household information so we can determine your discount level.
  3. Get your discount. Once approved, your discount applies to your KCS services. Our team will explain your fee level and answer any questions.
  4. Renew annually. Your sliding fee status is reviewed once a year. We will help you update your information when it is time to renew.
How much will I pay?
Your fee depends on your household income and size. After you apply, our team will assign you a discount level and explain your costs. Many patients pay a significantly reduced rate.
What if I cannot afford even the discounted fee?
No one is turned away for inability to pay. Talk to our team and we will work with you to make sure you can access care.
Does the sliding fee scale cover all services?
The sliding fee scale applies to most KCS services, including medical, dental, counseling, and more. Our team can confirm coverage for specific services.
How often do I need to reapply?
Your sliding fee status is reviewed once per year. We will help you renew when it is time.
