KCS Community Health is seeking a CalAIM Justice-Involved (JI) Lead Care Manager to provide comprehensive, person-centered care management to justice-involved re-entry members with complex medical, behavioral health, and social needs. You will serve as the primary coordinator across pre-release and post-release settings, working with correctional partners, healthcare providers, county entities, and community-based organizations to ensure continuity of care and successful re-entry outcomes.

• Serve as the single Lead Care Manager and primary contact coordinating medical, behavioral health, substance use, and social services for assigned members
• Conduct comprehensive assessments and develop, monitor, and update individualized re-entry care plans before and after release
• Facilitate warm handoffs between pre-release and post-release care settings, providers, and care managers
• Manage a caseload of roughly 20 to 25 members, adjusted to program and operational needs
• Coordinate communication among members, correctional partners, providers, county agencies, managed care plans, and community organizations
• Advocate for members to remove barriers to care and support stabilization during re-entry
• Apply motivational interviewing, trauma-informed care, and harm-reduction approaches to sustain engagement
• Track member outcomes and contribute to program evaluation, reporting, and quality improvement
• Maintain accurate, timely, compliant documentation in the EHR and required systems

• Bachelor's degree in social work, public health, healthcare management, or a related field
• 2+ years of care management for individuals with complex medical, behavioral health, and social needs

• Ability to work independently and within multidisciplinary teams

This is a salaried, full-time role of about 40 hours per week. You may occasionally work beyond standard hours to meet member needs, respond to urgent situations, or attend required trainings and meetings.

The Therapist I conducts mental health assessments, provides crisis intervention, and manages case support under clinical supervision. You will develop treatment plans, deliver both individual and group therapy, and coordinate patient needs. You will collaborate on complex cases and keep accurate clinical documentation.

• Provide diagnostic mental health assessments to patients under supervision
• Provide crisis intervention and case management support to assigned patients
• Develop treatment plans, monitor progress, and follow up as clinically indicated with supervisory oversight
• Deliver clinical therapeutic services, including individual and group therapy
• Coordinate patients' medication needs with psychiatric, case management, and medical resources
• Work closely with the consulting psychiatrist on complex patient cases
• Resolve barriers to care through follow-up and collaboration with KCS staff and other providers
• Maintain accurate, timely clinical documentation consistent with agency standards
• Follow all organizational policies and compliance requirements, including HIPAA and mandated reporting

• Master's degree in Social Work (MSW) or Marriage and Family Therapy (MFT)
• Current registration as an AMFT or ACSW in the State of California
• Experience with psychosocial assessment, DSM-based diagnosis, individual and group therapy, and crisis intervention
• Strong interpersonal, written, and oral communication skills
• Ability to navigate an electronic medical record and use technology effectively
• Criminal background check and clearance

The Facilities Assistant keeps KCS facilities running smoothly and well maintained across all our locations. You will handle routine maintenance, inventory, vehicle servicing, mail, and basic clerical support, and respond to repair requests. The role also includes general cleaning, minor repairs, and maintaining safety systems to meet health standards.

• Assist with the daily operations and maintenance of the health center's facilities
• Perform routine maintenance such as cleaning, minor repairs, and equipment inspections
• Transport equipment and packages to and from destinations using agency vehicles
• Oversee supply inventory across all locations, keeping medical equipment and office supplies stocked
• Sort and deliver mail, and maintain physical filing systems and records
• Manage the upkeep and servicing of company vehicles to keep them running reliably
• Respond promptly to maintenance requests and help coordinate response during facility emergencies
• Handle minor fixes, yard upkeep, installations, and refurbishments
• Inspect and troubleshoot equipment and safety systems, reporting issues to the facilities manager

• Valid driver's license, at least 2 years of driving experience, and a record that meets KCS vehicle insurance requirements

The IT Help Desk Representative provides technical support and assistance to staff, keeping our IT systems and infrastructure running efficiently. You will bring strong problem-solving skills, clear communication, and a commitment to high standards of service at our headquarters.

• Log support calls and run diagnostics and troubleshooting by phone, email, and ticket system
• Provide clear guidance to KCS hardware and software users
• Escalate complex IT issues to higher-level specialists for resolution
• Educate staff on IT best practices, system usage, and troubleshooting techniques
• Diagnose and resolve hardware, software, and network issues in a timely manner
• Install and configure PCs, printers, peripherals, operating systems, and network settings
• Maintain accurate records of issues and resolutions in the ticketing system
• Assist with maintaining security software and protecting KCS's confidential information and assets

• Associate degree in Information Technology, Computer Science, or a related field preferred
• Proven experience as an IT Help Desk Technician or in another customer support role
• Strong understanding of computer systems, mobile devices, and other tech products
• Proficiency in Windows environments and Microsoft Office Suite
• Familiarity with network and security concepts, including TCP/IP, DNS, DHCP, and firewalls
• Ability to lift and carry 20 to 50 pounds of equipment with or without reasonable accommodation
